  • If you have been instructed by your school to use the Primary.Health reporting platform, use that form instead. Do not report on both this and the Primary.Health reporting form.

    • This form is to be used by individuals who attend or work in a Marin County school to report the results of COVID-19 testing done in the home to the Marin County Public Health Department.
    • Please do not complete this form if you received testing by a medical provider or COVID testing site. Only report testing you administered yourself with the kits distributed through your school.
    • The information you report will be used by Marin County Public Health in the same manner as tests conducted by a medical provider or COVID testing site. We will use information you provide to accomplish our duties as a Public Health authority, if essential to the completion of the objectives of the County’s COVID-19 testing or surveillance program and as required by law.
    • We will report results to relevant State and schools testing tracking systems as required by Title 17, California Code of Regulations (CCR) 2500, 2593, 2641.5-2643.20, and 2800-2812 Reportable Diseases and Conditions.
